When you need support and send a request of support to fertsoftsupport@omda.com or by adding a support ticket here on the support portal it is important to identify the correct record the support ticket concerns. The following identifying numbers can be used to identify the record you want help with.
Patient
A patient record can be identified by the Patient ID.
In the picture above the following numbers are displayed:
- Patient ID - 16 - Red
We never require or ask for the social security number or name of the patient. Please refrain from sending this information.
Treatment
A treatment has several numbers that can identify it. There is a Treatment ID, a treatment number and you can also send the Patient ID for verification.
In the picture above the following numbers are displayed:
- Patient ID - 15999 - Red
- Treatment ID - 28300 - Orange
- Treatment number - 16063 - Green.
We ask you to send the Treatment ID and the Patient ID for verification, if you would like you can also include the Treatment number.
Sperm analysis
A sperm analysis is mainly identified by the Sperm analysis ID but can be verified by multiple other numbers.
In the picture above the following numbers are displayed:
- Sperm analysis ID - 8443 - Blue
- Patient ID - 17719 - Red
- Treatment ID - 33971 - Orange
We ask you to send us the Sperm analysis ID and the Patient ID for verification.
